How is someone nominated for Heroes of Hope™ Living with CF?
Heroes of Hope Living with CF can be nominated by family members, friends, healthcare professionals, teachers, and community members.
To nominate your own hero:
- You may download the nomination form here. Nomination forms may also be available at Cystic Fibrosis
Care Centers throughout the United States.
Please submit the completed form to:

or Fax: (888) 895-0877 - Inform the individual or guardian that you are nominating him or her for the award and obtain a signature verifying that he or she is aware of the nomination.
- Obtain a signature from the nominee's healthcare professional confirming that the nominee has CF.
- Nomination Process: After you submit the nomination form, the form will be reviewed by a panel, who will make the final selection, based on the following criteria: attitude, CF health maintenance, determination, service, and achievement. Some participants may receive a call for an interview, and then the final selection will be made by the panel.
Stories about Heroes of Hope Living with CF do not need to mention the use of any Genentech products.
The nomination form will remain active for three months. If the nominee has not been chosen within three months from the original submission date, the candidate's name and nomination must be resubmitted for consideration.
Any medical information you provide will be held in strict confidence by the Heroes of Hope™ Living with CF program panel members and Genentech, Inc. Any information you provide will be utilized only for the purposes of evaluating your nomination and communicating with you regarding your nomination and/or selection. We will not sell your information or share your information with any other company or organization aside from the vendors we have hired to manage communications with you regarding the Heroes of Hope™ Living with CF program.
