Who can nominate a Hero?
Heroes of Hope™ Living with CF can be nominated by family members, friends, healthcare professionals, teachers and community members.

Who qualifies to be Heroes of Hope™ Living with CF?
Persons of all ages with the diagnosis of cystic fibrosis who are living in the United States are eligible for nomination. Although this program is brought to you by Genentech USA, Inc., use and/or purchase of any Genentech products is not required for nomination or selection.

What does a nominee receive if he or she wins the Heroes of Hope Award?
The nominee receives the honor and recognition of being selected, among a competitive pool of candidates, for being an inspiring person living life to the fullest, in front of the CF community. For this honor, the recipient will be featured on the Heroes of Hope Web site and may be featured in local media and on cystic fibrosis advocacy group Web sites. The recipient receives a personalized, engraved crystal award, a certificate, a banner and a t-shirt. There is no monetary compensation for being recognized as a Hero.

I want to surprise my nominee if he or she wins the Heroes of Hope Award. Can I nominate a Hero without telling him or her?
The program requires that the nominee gives consent to be nominated. The selection panel seeks candidates who are willing to talk openly about having cystic fibrosis in an interview, so having the nominee's consent confirms this fact. The program also requires that the nominee and his/her parent or guardian give consent for the release of medical information on the nomination form.

My nominee does not speak English. Can I still nominate him or her?
Please complete the nomination form in English. However, the Heroes of Hope Program will provide a translator for the interview if this nominee is selected as a Hero.

What should I do if I haven't heard anything from Heroes of Hope?
If the Heroes of Hope Program has received your nomination, you will receive a confirmation card in the mail. If you haven't received any correspondence from the Heroes of Hope Program, the program may not have received your nomination. From January 2008 to June 2008, the Heroes of Hope Program re-organized and the fax number has changed. It is important that you keep a copy of your nomination form for your records and for purposes of resubmission. Please re-submit your nomination form again to the address or fax number below:

Can more than one member of a family receive the award?
Yes. Separate nomination forms are required per individual. Please be as thorough as possible when describing the unique attributes of each individual nominated. The panelists will only select one recipient per month. Individual interviews will be conducted with each Hero selected.

How many times can I submit a nomination?
There is no limit on the number of times a nomination for the same individual can be re-submitted.

Why does the Heroes of Hope Program need to know the name of my CF doctor and care team?
The panel will only contact your CF care team to obtain confirmation that the nominee has the diagnosis of cystic fibrosis. The program will not discuss the nominee's personal and medical information with the CF care team.

What can I do to maximize the chances that my nominee receives this award?
Please see Tips for Your Nomination Form (below) for information about filling your nomination form. Also, if you have not received notice that your nomination was received, please follow up with the program to track the status of your nomination. If your nominee hasn't been selected in over 12 months, please re-submit your nomination. The pool of competitive candidates changes on a regular basis, and your nominee may be selected at a different point in time.

How often is a Hero of Hope selected?
A Hero of Hope is selected every month.

Tips for Your Nomination Form

  • Please fill out your nomination legibly in black or blue ink so that it is easy to read. Faxes can make handwriting difficult to read.
  • Please be as thorough as possible with your nomination form. The selection panelists want as much information as possible to get an idea of the nominee's attitude, determination, compliance, service and achievement.
  • If the nominator writes "See Attached" on the nomination form, the selection panel will only accept a maximum of two pages (1000 words) of extra information (ex: a newspaper article, extra letter from a loved one, written material from the nominee). Additional pages will not be accepted. Links to Web pages and blogs will be accepted.
  • Panelists are interested in clearly knowing what kind of effort the nominee makes to stay healthy and whether he or she is compliant with the health maintenance regimen.
  • Anecdotes or direct quotes are accepted to portray a nominee's attitude and determination.
  • Please make sure all contact information is clearly completed, especially e-mail and phone contact information, and signed.

Stories about Heroes of Hope™ Living with CF do not need to mention the use of any Genentech products.

The nomination form will remain active for three months. If the nominee has not been chosen within three months from the original submission date, the candidate's name and nomination must be resubmitted for consideration.

Any medical information you provide will be held in strict confidence by the Heroes of Hope™ Living with CF program panel members and Genentech USA, Inc. Any information you provide will be utilized only for the purposes of evaluating your nomination and communicating with you regarding your nomination and/or selection. We will not sell your information or share your information with any other company or organization aside from the vendors we have hired to manage communications with you regarding the Heroes of Hope™ Living with CF program.
